The Modern Male Patient: Why More Men Are Choosing Botox

March 23, 2025

At MyAesthetics, we’re all about challenging stereotypes, especially when it comes to medical aesthetics. While cosmetic injectables and other medical-grade treatments have long been popular among women, the growing number of men seeking these services is still under the radar. Some men may think of these treatments as “unmanly,” but the truth is, we use these services to support and enhance a more masculine appearance. While gyms are packed with men working to lose fat and build muscle for a more defined physique, many are overlooking the one part of the body that’s most visible: the face.

Though roughly two-thirds of aesthetic service clients are female, recent years have seen a growing number of men embracing these treatments. Particularly, men are increasingly drawn to minimally invasive procedures like injectables. Between 2000 and 2018, the American Society of Plastic Surgeons reported a nearly 30% rise in men seeking cosmetic procedures, alongside a significant 45% drop in invasive surgeries among men. This trend sets the stage for the surge in minimally invasive procedures, which have skyrocketed by 72% during the same period, according to the ASPS. Interestingly, a study published in the Journal of the ASPS found that while the ratio of female to male injectable patients was 9:1, men were only outnumbered 2:1 when it came to interest in cosmetic treatments. The rise in male interest can be attributed to factors like better products and services, increased workplace competitiveness, social media influence, and greater social acceptance.

Ironically, men may actually be at a higher risk for extrinsic aging than women, making them ideal candidates for certain treatments. Men tend to spend more time outdoors and are less likely to use sunscreen or other preventive measures. They also typically have stronger facial muscles, which can lead to quicker development of fine lines and wrinkles. But it’s not just lifestyle habits that differ—there are also distinct evolutionary differences that define facial masculinity. Let’s take a closer look at these traits through the structural foundation of the face: the skull.

As it turns out, the saying “thick-headed” is true, quite literally! Men have skulls that are 25% larger than women’s, with thicker bones, more prominent brow ridges, larger eye sockets, and more angular jawlines and chins, all due to higher levels of testosterone. Generally, masculine faces are more boxy, whereas female faces tend to be oval or heart-shaped. Evolution has taught our brains to subconsciously associate these masculine features with strength, dominance, and power. However, as we age, hormone changes cause bone resorption, and gravity contributes to the sagging of facial soft tissues. Additionally, sun damage and repetitive facial expressions lead to the formation of wrinkles.

This is where non-invasive cosmetic procedures come into play. While some treatments are the same for both men and women, the desired outcomes are often very different. That’s why it’s crucial to choose a provider who understands the unique facial differences between genders, especially for male patients. In my experience, injectable fillers for the jawline and cheeks are among the most rewarding procedures, as they add youthful definition to the face. When enhancing masculinity, the goal is to achieve a stronger, wider, and more chiseled jawline and chin, with cheek projection closer to the midline of the face. For women, the focus would be on a more sculpted jawline, a rounded chin, and cheek projection extending more laterally.

Of course, this varies depending on the patient’s specific goals. While a one-size-fits-all approach might work in some practices, at MyAesthetics, we tailor each treatment to the individual.
